Position Summary

The Certified Nursing Assistant (CNA) provides personal care and supportive services to hospice patients in their homes, assisted living facilities, nursing facilities, or other care settings under the direction and supervision of a Registered Nurse. The CNA plays a vital role in ensuring patients remain comfortable, safe, and cared for with dignity and respect.
Essential Duties and Responsibilities

Patient Care

  • Provide personal care services including bathing, grooming, oral hygiene, dressing, toileting, and skin care.
  • Assist patients with mobility, transfers, positioning, and ambulation.
  • Help patients maintain comfort and dignity throughout the end-of-life journey.
  • Assist with feeding and hydration as directed by the care plan.
  • Observe and report changes in patient condition, behavior, physical status, or environment to the supervising nurse promptly.
  • Monitor and document vital signs as assigned and report abnormalities.
  • Maintain a safe and clean environment for patients.

Family Support

  • Provide emotional support and compassionate interaction with patients and families.
  • Educate caregivers on basic personal care techniques as directed by the nursing team.
  • Support family members by helping maintain patient comfort and dignity.

Documentation & Communication

  • Complete required documentation accurately and timely according to company policy and regulatory requirements.
  • Communicate effectively with members of the interdisciplinary hospice team.
  • Participate in team meetings and care conferences as requested.
  • Follow the established plan of care and report any concerns to the supervising nurse.

Compliance & Professional Standards

  • Maintain patient confidentiality and comply with HIPAA regulations.
  • Adhere to hospice policies, infection control standards, and safety protocols.
  • Comply with federal, state, and local regulations governing hospice care.
  • Maintain current certification and required training.

Qualifications Required

  • Current Certified Nursing Assistant (CNA) certification in the state of Georgia.
  • High school diploma or equivalent.
  • Minimum of one (1) year of CNA experience in healthcare settings.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to travel within assigned service area.
  • Basic knowledge of patient care practices and infection control procedures.
  • Strong communication and interpersonal skills.

Preferred

  • Previous hospice, home health, palliative care, long-term care, or geriatric experience.
  • CPR certification.
  • Experience working with terminally ill patients and their families.

Knowledge, Skills, and Abilities

  • Compassionate, patient-focused approach to care.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Strong observation and reporting skills.
  • Excellent time management and organizational abilities.

  • Ability to lift, transfer, and reposition patients with or without assistance.
  • Ability to stand, walk, bend, stoop, kneel, and reach frequently.
  • Ability to lift up to 50 pounds and push/pull equipment as needed.
  • Visual and auditory acuity sufficient to monitor patient conditions and communicate effectively.

Work Environment

  • Patient homes, assisted living facilities, nursing facilities, and other residential care settings.
  • Frequent travel within the service area.
  • Exposure to varying environmental conditions and infectious diseases.


Position Summary

The RN is responsible for coordinating and managing the overall plan of care for hospice patients. This role provides skilled nursing assessments, symptom management, patient and family education, and care coordination to ensure patients receive the highest quality, comfort-focused care. The RN serves as a key member of the interdisciplinary team and advocates for patients and families throughout the hospice journey.
Essential Duties and Responsibilities
Clinical Assessment & Care Planning

  • Conduct comprehensive patient assessments and ongoing evaluations of physical, emotional, psychosocial, and spiritual needs.
  • Develop, implement, and update individualized hospice plans of care.
  • Monitor patient status and adjust care plans as needed to meet changing needs.
  • Coordinate physician orders and ensure timely implementation of treatment plans.
  • Manage pain and symptom control utilizing hospice and palliative care best practices.
  • Identify and respond to urgent patient care needs, including after-hours support as assigned.

Case Management

  • Serve as the primary clinical coordinator for assigned hospice patients.
  • Coordinate care among physicians, nurse practitioners, social workers, chaplains, hospice aides, volunteers, and other healthcare providers.
  • Ensure all services are delivered according to the patient's plan of care.
  • Facilitate patient admissions, recertifications, transfers, discharges, and end-of-life care transitions.
  • Participate in interdisciplinary team meetings and case reviews.

Patient & Family Education

  • Educate patients and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
  • Provide emotional support and guidance to patients and families during difficult transitions.
  • Assist families in understanding hospice philosophy, goals of care, and available resources.
  • Support advance care planning discussions when appropriate.

Documentation & Compliance

  • Maintain accurate, timely, and complete clinical documentation in the electronic medical record.
  • Ensure compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and company policies.
  • Complete required assessments, care plans, visit notes, and physician communications within established timeframes.
  • Participate in quality improvement and performance improvement initiatives.

Leadership & Collaboration

  • Supervise and provide direction to Certified Nursing Assistants (CNAs), Licensed Practical Nurses (LPNs), and other support staff as assigned.
  • Collaborate effectively with the interdisciplinary team to deliver coordinated patient care.
  • Serve as a clinical resource for patients, families, and team members.
  • Promote a culture of compassion, accountability, and excellence.

Qualifications Required

  • Current and unrestricted Registered Nurse (RN) license in the state of Georgia.
  • Graduate of an accredited nursing program.
  • Minimum of two (2) years of nursing experience.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to travel throughout assigned service areas.
  • Strong clinical assessment and critical thinking skills.
  • Proficiency with electronic medical records and documentation systems.

Preferred

  • One (1) year of hospice, palliative care, home health, oncology, geriatrics, or related experience.
  • Experience with Medicare hospice regulations and compliance requirements.
  • Previous case management experience.

  • Strong understanding of hospice philosophy and end-of-life care principles.

Position Summary

The Hospice LPN provides direct nursing care and supportive services to hospice patients under the supervision of a Registered Nurse. The LPN assists with patient assessments, symptom management, medication administration, and patient/family education while supporting the goals of comfort-focused care. This role is essential in helping patients remain comfortable and supported throughout their hospice journey.

Essential Duties and Responsibilities

Direct Patient Care

  • Provide skilled nursing care to hospice patients in homes, assisted living facilities, nursing facilities, and other care settings.
  • Perform routine nursing assessments and report findings to the supervising RN.
  • Monitor and document patient condition, pain levels, symptoms, and response to treatment.
  • Administer medications and treatments as ordered by the physician and directed by the RN.
  • Assist with wound care, catheter care, ostomy care, and other skilled nursing procedures within the LPN scope of practice.
  • Observe for changes in patient condition and promptly communicate concerns to the RN Case Manager.

Patient & Family Support

  • Educate patients and caregivers regarding medications, symptom management, and basic care needs.
  • Reinforce hospice philosophy and care plan goals established by the RN and interdisciplinary team.
  • Provide emotional support and compassionate care to patients and families facing end-of-life challenges.
  • Assist caregivers in developing confidence and competence in providing daily care.

Care Coordination

  • Collaborate with RNs, physicians, CNAs, social workers, chaplains, and other members of the interdisciplinary team.
  • Participate in patient care conferences and team meetings as requested.
  • Assist in implementing and monitoring individualized plans of care.
  • Support continuity of care through effective communication and follow-up.

Documentation & Compliance

  • Complete clinical documentation accurately and promptly according to agency policy and regulatory requirements.
  • Document nursing interventions, patient responses, medication administration, and caregiver education.
  • Maintain compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and company policies.
  • Protect patient privacy and maintain HIPAA compliance.

Quality & Safety

  • Follow infection control and safety procedures at all times.
  • Participate in quality improvement initiatives and ongoing staff education.
  • Maintain current licensure and required certifications.
  • Report incidents, safety concerns, and patient care issues according to company policy.

Qualifications Required

  • Current and unrestricted Licensed Practical Nurse (LPN) license in the state of Georgia.
  • Graduate of an accredited practical nursing program.
  • Minimum of one (1) year of nursing experience.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Strong clinical observation and communication skills.
  • Ability to work independently while collaborating with a multidisciplinary team.

Preferred

  • Hospice, palliative care, home health, long-term care, oncology, or geriatric nursing experience.
  • CPR certification.
  • Experience with electronic medical records (EMR).
  • Knowledge of hospice regulations and end-of-life care principles.
